South Routt Medical Center Expansion & Renovation Project

Project Overview

South Routt Medical Center is maintaining project readiness while required reviews are underway for the proposed expansion and renovation of its facility at 300 E. Main Street. The project will strengthen access to primary care, urgent care, behavioral health, laboratory services, telehealth, and specialty care for the South Routt community.

The project includes renovation of the existing building and construction of an approximately 880-square-foot addition to expand clinical capacity, improve patient flow and privacy, and enhance accessibility.

Official public hearing notices and procedures will be issued by the Town of Oak Creek.

Why This Project Matters

South Routt residents rely heavily on local access to care due to long travel distances, winter road conditions, and limited transportation options. A recent feasibility study confirmed:
• A 1.6 FTE shortage in primary care providers
• A 4.5 FTE shortage in behavioral health providers
• The current building is undersized for today’s patient volume
• Space limitations restrict provider recruitment and service expansion
The expansion will support urgent care, behavioral health services, telehealth, specialty outreach providers, expanded laboratory services, and improved patient support areas. The project is designed to improve healthcare access while supporting the long-term sustainability of local healthcare services.

The proposed project includes:
• Six exam rooms
• One urgent care room
• One procedure room
• Dedicated blood draw and infusion space
• Expanded laboratory
• Telehealth exam room
• Provider and Medical Assistant workroom with ten workstations
• Two private offices
• Larger, more private registration area
• Staff restroom and lounge
• Improved circulation and patient flow
• Nineteen paved parking spaces, including four ADA-accessible spaces

Project Funding
The recommended project budget is $3.6 million.
South Routt Medical Center has secured federal and state grant funding to support planning, design, and construction of the project, including:

$307,000 – Colorado Office of Economic Development & International Trade (OEDIT)
Coal Transition Tier 1 Community Planning Grant
Supports feasibility studies, master planning, and conceptual design for healthcare infrastructure in a transitioning rural economy.

$1 Million – Colorado Department of Local Affairs (DOLA)
Energy Impact Assistance Fund
Provided funding for architectural and engineering development necessary to advance the project through design and construction readiness.

$2.3 Million – Health Resources & Services Administration (HRSA)
Federal Construction Support for Rural Health Access
Supports facility expansion to improve healthcare access and capacity in rural communities.

These investments represent a significant commitment to expanding healthcare access in South Routt while reducing the financial burden on local taxpayers.

In addition to grant funding, SRMC is pursuing philanthropic partnerships to support furnishings, fixtures, equipment, technology investments, and other project-related needs that will enhance patient care and long-term operational sustainability.

• Permitting: March 2026 – July 2027
• Public Bidding: Completed
• Construction Award: Pending Final Funding Review
• Construction Start: Anticipated Following Funding Completion

Project timelines are preliminary and may be adjusted based on permitting, funding, procurement, contractor availability, and construction conditions.

District Progress

Over the past two years, the District has completed:
• Community Health Needs Assessment
• Feasibility Study and Master Plan
• Architectural design and construction documents
• Site planning and civil engineering
• HRSA-compliant procurement and public bidding
• Financial modeling and operational planning
• Hired additional Physician to meet growing community need

The project is currently progressing through final review, funding coordination, and public approval processes prior to construction.

Current Project Status

South Routt Medical Center is currently engaged in planning, regulatory review, fundraising, and project readiness activities necessary to maintain grant eligibility and meet applicable project timelines. Current efforts include land use and entitlement processes, design coordination, and community engagement.

The project remains subject to Board direction, funding availability, required approvals, and completion of applicable procurement processes. No construction contract has been awarded at this time.
